Balanitis is one of those conditions patients often feel embarrassed to talk about — but it’s incredibly common, very treatable, and nothing to be ashamed of. At CallOnDoc, we see it every day in telehealth visits, and most cases resolve quickly with proper care.
This guide walks you through what balanitis is, why it happens, how to treat it, and when to follow up for more help.
Balanitis is inflammation of the glans penis (the head of the penis), and it can affect the foreskin as well if someone is uncircumcised.
Common symptoms include:
Redness or irritation
Itching or burning
Pain or sensitivity
Swelling of the foreskin
Discomfort with urination
A shiny or dry rash
White patches or discharge (yeast-related)
Odor (when bacteria or yeast overgrow)
It can range from mildly uncomfortable to significantly painful — but it is not dangerous when treated early.
Think of the area under the foreskin like a warm, moist environment — the perfect place for irritation or overgrowth of bacteria/yeast.
Common causes:
Candida overgrowth can cause bright red, inflamed skin with small red “satellite” dots.
Triggered by:
Harsh soaps
Body wash
Fragranced products
Condom lubricants
Sweat & friction
Tight clothing
Poor hygiene or overwashing
Less common but causes pain, swelling, mild odor, or sometimes discharge.
Eczema, psoriasis, or lichen sclerosus can mimic balanitis.
High blood sugar increases yeast growth — recurrent balanitis can be an early sign of uncontrolled or undiagnosed diabetes.
During your telehealth visit, we’ll ask:
What symptoms you’re having
When they started
Whether you’re circumcised or not
New soaps, detergents, or products
Condom or lubricant use
Recent sexual activity
Diabetes history or symptoms (increased thirst, urination)
Photo of the affected area (fully optional but extremely helpful)
You do not need an in-person exam to get treatment in most cases.

Topical antifungal cream (clotrimazole or miconazole)
Oral fluconazole for tougher cases
Low-dose steroid cream to reduce inflammation (only short-term)
We recommend:
Stopping scented products

Warm water rinsing only
Applying a short course of low-potency steroid cream
Avoiding sexual activity until healed
We may prescribe:
Topical antibiotic
Short course of oral antibiotics if needed
Specialized creams may be needed — we’ll guide you.
If symptoms are recurrent, we may recommend a diabetes screening through your primary provider or labs.
Proper care can speed healing and reduce recurrences significantly.

âś” Dry gently — tap, don’t rub
âś” Avoid lotions or scented products
âś” Avoid sex until symptoms resolve
âś” Wear loose cotton underwear
âś” Change out of sweaty clothing ASAP
âś” If uncircumcised: gently retract foreskin, rinse with warm water, and pat dry
Contact a provider immediately if you notice:
Severe swelling or inability to retract foreskin
Trouble urinating
Fever or spreading redness
Cracks or ulcers
Rapid worsening of symptoms
These may signal balanoposthitis, phimosis, or another infection that needs prompt treatment.
